Question  Reforms over other reforms...

I'm confused really about the "Excludes" and "Excluded by" under each reform. Example: installing Liberalism and then Anarcho-capitalism; installing Militarism and then Pacifism.
I read the descriptions and forums, however, I couldn't find a clear answer to this part of the game, especially since I see some states install some reforms that seems to contradict or exculde others...

"Excluded by" means that these reforms are not applicable if other reforms are in effect.

"Excludes" means that appliying this reform prevents the later application of other reforms.

Both cases provide information as to which reforms are exlucuded, or which reforms exclude them. Also note, it is possible to apply some reforms which are excluded by others but that, in themselves, do not exclude other reforms: For example you can attempt Universal Healthcare after Liberalism (or vice versa); etc. I recommend you experiment with doing such if you are interested -- it is a good idea to witness in-game effects first hand before taking a determined path.
